I’ve recently been thinking about the importance of celebration or ways to add more celebration to your life. Finding new ways to celebrate seems especially important now, given the grim state of the economy and the world. We need more ways to step outside the mundane everyday reality to have more fun and feel better about ourselves and our experiences everyday.

The value of celebration has long been recognized in every culture, going back to the beginnings of human history and even before. It is closely linked with the use of ritual and play, which even many animals do. A ritual is a regularly performed action, and often it is done as part of a religious service or personal spiritual practice. Celebration is often incorporated in such religious rituals, such as when a prayer session or meditation concludes with a sense of joy and liberation. Likewise, play, which involves engaging in a free spirited fun activity, can incorporate a celebration or conclude with one. An example is when a game ends in a victory which calls for a celebration.

Celebrations can also give special recognition and fun to almost anything – from birthdays and holidays to the successful completion of any kind of tasks in business or at work.

You might think of celebrations as those special occasions that add a sense of excitement, awe, joy, or other types of heightened emotional release. They are a way to brighten and add meaning to your life, and they bring people together in a spirit of community and bonding. Without celebration life would be truly drab and monotonous – just doing a series of routine tasks day after day. It would be a little like being on a factory line that keeps going, going, and going and never ends. You would feel bored and unstimulated, living but not really alive. But a celebration, even a short one lasting just a few minutes, helps to recharge the mind and the soul.

Hence, the power of all sorts of celebration throughout history. There are celebrations to mark the passage from one stage of life to another – such as baptisms to welcome a new child to the family and community, birthdays, coming of age ceremonies like barmitzvahs and fraternity hazings, marriage ceremonies, seasonal holidays like Easter and Christmas, and Saints Days in the Catholic Faith. There are days when one can be someone else, such as during Mardi Gras and Halloween. There are celebrations to welcome newcomers to a group, organization, company, or neighborhood. There are parties to support or celebrate the success of a team, like tailgate parties and victory parties. Other parties are for achievements and promotions, still others for retiring or leaving a group.

In short, almost any event you can think of has a celebration associated with it – and there are special celebrations around the world associated with a particular industry, such as fleet days when the boats launch; launch parties for when a new company begins; and more.

Individuals, families, and blocks of neighbors may create their own celebrations, too, for different occasions. For instance, you might create a personal celebration to reward yourself for achieving some goal; your family might arrange to celebrate a special family day, such as by having a family picnic, graduation party, or outing at the beach. Your neighbors might organize a block party or become part of a network of block parties, such as National Night Out, which is sponsored by law enforcement to encourage neighbors to bond together and protect one another against crime.

For example, you can give yourself a quick reward at the end of completing a task or a certain amount of work. You can take a stretch or exercise break and imagine that you are celebrating some kind of achievement while you do. If you are doing routine work that doesn’t involve much concentration, you might imagine yourself engaging in some fun activity.

Or you might find ways to make work more fun for everyone, such as by having a party where people decorate their workplaces. Other possibilities include organizing a special dress-up day, creating a bingo awards game for people’s work, taking a goofy break to unwind, having a time at a meeting for people to brag about themselves, and beginning or ending a meeting with an inspirational story.

Still other possibilities are having a creative gift exchange, organizing a humorous theme contest or event, setting up an office skit or poetry slam party, and having a fashion show. Or try devising a workplace scavenger hunt, having a diversity day celebration, and organizing a creativity day where team members come up with creative uses for a series of objects.

Some other ideas for fun celebrations include creating a match-up game to help people learn about each other, having a rewards and recognition contest, using humorous gifts for office celebrations, and finding novel ways to recognize employees. Some more suggestions are putting on a surprise party for a special occasion, organizing a come-as a character day, creating a be-a-kid for a day event, and organizing a theme party featuring a fun place, such as a Down Under Australia party or Polynesian luau.

If you travel, you will find opportunities to participate in a wide variety of celebrations in different communities throughout the U.S. and the world. Just pick up the local daily or weekly newspaper and see what’s going on. For example, when I was in Southern California, one of the special celebrations I attended was a party to look for and welcome the grunions, a small fish that arrives in mass on the beach at certain times of the year to spawn. Another event was a Harvest Festival put on by a local college which featured a walk through a mysterious corn maze where costumed figures jumped out from the corn to startle everyone. And one of my favorites was a monthly drumming ceremony on the playa, where people came with their drums and rattles and danced and chanted around a bonfire.

In short, there are all sorts of ways to add celebration to your life – from what you do individually to what you do with friends, family, neighbors, your whole community, and the nation.

And you can not only engage in celebrations with others, but you can use your imagination to create personal celebrations in your mind. In turn, such celebrations not only add fun and excitement to your life, but they can help you become close to and bond with others and add meaning to your life. Also, when you use your imagination to come up with and experience celebrations, you are exercising and expanding your creativity, as well as stimulating your mental abilities, particularly your ability to visualize and plan. The type of cast used when fishing varies according to the conditions you are fishing in. The most common cast is the forward cast, where the angler whisks the fly into the air, back over the shoulder until the line is nearly straight, then forward, using primarily the forearm. The objective of this motion is to “load” the rod tip with stored energy, then transmit that energy to the line, resulting in the fly line being cast for an appreciable distance. Casting without landing the fly on the water is known as ‘false casting’, and may be used to pay out line, to dry a soaked fly, or to reposition a cast. Other casts are the roll cast, the single- or double-haul, the tuck cast, and the side- or curve-cast.

Dropping the fly onto the water and its subsequent movement on or beneath the surface is one of fly fishing’s most difficult aspects according to experts. In short, the angler is attempting to cast in such a delicate way that the line lands smoothly on the water and the fly appears as natural as possible – a difficult task for many. At a certain point, if a fish does not strike, depending upon the action of the fly in the wind or current, the angler picks up the line to make another presentation – all the while keeping trying to keep their moves very delicate. When fly fishing it is extremely important to remember patience and fluid line movement.

On the other hand of being patient is getting that so desired catch. If a fish strikes, the angler pulls in their fly fishing line while raising the rod tip. This “sets” the hook in the tasty fishs mouth. The fish is played either by hand, where the angler continues to hold the fly line in one hand to control the tension applied to the fish, or by reeling up the slack in the line and then using the hand to act as a controlling drag on the reel. Some fly reels have an adjustable, mechanical drag system to control line tension during a fish’s run which comes in very handy to inexperienced and seasoned fly fisherman.

Of course, no amount of reading online can really teach you what you need to know for a fly fishing extravaganza. In fact many experts agree that nothing can replace professional, qualified instruction. When Your Health Insurance Plan Won’t Pay

The very nature of managed care health insurance plans increases the likelihood of a legitimate health insurance claim being denied. Bear in mind that managed care (health maintenance organizations, or HMOs, and preferred provider organziations, or PPOs) exist for the purpose of controlling costs for the health insurance company. Many health care procedures, surgeries, durable medical equipment and drugs, particularly the more expensive ones, require prior authorization from the health insurance plan before the plan will pay. Claims are reviewed to determine “medical necessity” of the claim. Health care services or products deemed “not medically necessary” will almost certainly be denied for payment by the health insurance plan.

Health insurance companies do make mistakes, however, and it’s certainly possible that a covered expense will be denied. What recourse does the health plan member have when one disagrees with the decision of the health plan? Here are some steps to take in dealing with a denial of payment.

1. Review the explanation of benefits (EOB) sent to you from the health insurance company. The EOB should state what services or goods were billed and briefly why benefits were denied.

2. Review your particular health insurance policy. What benefits does the health insurance policy state for the particular service or product? Should the claim be covered according to the policy?

3. Does the health plan have special criteria to be met in order for an particular expense to qualify as “medically necessary” and be considered a covered expense? For example, many managed care plans will cover drugs on their formulary. Other, nonformularly drugs may not be covered at all, or may be covered only if the formulary drugs have been tried and failed. An expensive MRI procedure may only be covered if certain symptoms are present. Check your policy to determine whether the expense qualifies as “medically necessary” by the health insurance company. Your health care provider must submit sufficent documentation to the health insurance plan to justify the need for the expense.

4. Is the health care provider “in-network” (contracted) with your health insurance plan? If not, does your managed care plan cover “out-of-network” (non-contracted) providers? Most HMO plans do not cover “out-of-network” providers; many PPOs will pay for services by “out-of-network” providers, but usually at at lower rate than paid to “in-network” providers.

If, after reviewing the health insurance policy and the EOB, you feel that the claim should have been a covered benefit by the insurance company, you should first request in writing that the insurance company provide you with the information that they used to base their denial of benefits. The health insurance company is required to provide you with this information on request. Review this information carefully. Many times the health insurance company was not provided with appropriate or sufficient documentation from the provider to justify the claim. If this is the case, contact the provider and request that they submit more medical records that support the claim for benefits. It may also be helpful for the provider to write a letter to support the claim in addition to the medical records. Your claim may be resolved in this manner.

All health insurance companies have a process in place by which plan members can appeal the decisions of the health insurance company. If providing further documentation does not resolve the dispute, then an appeal must be filed with the health insurance plan. Your provider may help you with this, and they may not. Read the member handbook and/or policy and follow the procedure for appealing the denial of the claim. Be prepared to submit more documentation to support your appeal. Keeping a record of all interactions with the insurance company is vital. Record all phone conversations and include the name of the person you spoke with, a brief summary of the conversation, and the date and time. File all correspondence sent and received, and have it readily accessible.

Bottom line is that health insurance plans are “for-profit” entities; in business to make money. They look for reasons not to pay. Indeed, their goal is to not pay, increasing their profits and keeping costs down for the members. It’s up to you to ensure that legitimate claims for covered benefits are paid.

WoW Cooking Guide

My favorite online game to play is WoW (World of Warcraft) and that is why I am writing this short WoW cooking guide article. I am going to show you a brief overview of the cooking skill in the game WoW. When I say WoW I am referring to the MMORPG World of Warcraft and that is how I will be referring it through out this article.

OK the first thing you will have to do to start cooking in WoW is to visit a cooking trainer to learn the skill. Most of the major cities in the game have cooking trainers and you can get directions to them by asking a guard in the city. Once you find the trainer you will want to learn the apprentice cooking skill and all the recipes you can learn at the moment. You will also want to learn the skill to build a fire so you can cook just about any where you want. This can come in handy if you are out farming material to cook with and are not near a fire source to cook with. You will need to carry a flint and tender and some simple wood in your inventory to build a fire after you learn the skill.

OK now that you have learned all your skills you can buy some ingredients from a cooking supplies vendor and you should be ready to start cooking. You can also buy the flint and tender and the simple wood to build your own fire from the cooking supplies vendor. Just remember that when you reach certain levels you will be able to learn new recipes and you will have to visit the cooking trainer to learn these new recipes.

To gain skill points you will need to make items that you can cook and this will raise your skill points. You can see these items by opening your cooking interface and the cooking icon to do this should be in your spell book. When you open this interface you will notice some things are color coded with orange, yellow, green, or maybe even gray. As you cook items these will change as your skill level goes up. You will want to try and cook items that are colored orange to guarantee skill points that way will gain a point for each item cooked. If you cook items that are yellow or green you are not guaranteed a skill point. If you cook items that are gray you will gain no skill points because your cooking skill level is to high for these items.

There is six different level classes in WoW cooking that you will have to gain as you progress. These different levels are attained by reaching certain skill levels in cooking. You will learn these levels from a cooking trainer once you reach these skill levels. Here is a brief over view of the different levels:

  1. 1-75 Apprentice (Visit trainer)
  2. 75-150 Journeyman (Visit trainer)
  3. 150-225 Expert (Purchase the book called Expert Cookbook from a NPC vendor)
  4. 225-300 Artisan (must complete quest)

In order to learn the expert cooking skill you will need to purchase the Expert Cookbook. You can get this book in Shadowprey Village in Desolace (Horde) or at Silverwind Refuge in Ashenvale (Alliance). There should be a NPC vendor in these two areas to purchase it from.

In order to learn the artisan cooking skill you must complete some quests. The cooking trainer named Zamja in Ogrimmar (Horde) is where you should be able to start the quests. If you are alliance a NPC named Daryl Riknussun in Ironforge should be able to help. You will need a 225 Cooking skill to start the quests and a minimum player level of 35 to learn artisan cookin.

You can learn master cooking from a book sold by cooking trainers in Hellfire Peninsula. The horde can buy their book from a NPC named Baxter in Thrallmar. The alliance can get there book from Gaston in Honor Hold.

You can learn the grand master skill from cooking trainers in Northrend. Just visit your factions cooking trainer.

What Coleman Camping Gear Should You Take on Your Next Camping Trip?

There are many different Coleman camping gear essentials that you can take on your camping trip to make your camping experience one that you will enjoy.

  • A lightweight torch so you see what you are doing at night if you cannot light a fire where you will be camping.

2. National Park or Camp Grounds Camping:

With this type of camping, you can usually get to it by car. When you have a car to bring all your camping needs then you don’t have to worry about only bringing what you can fit in a backpack. If you’re camping in a national park or a camping ground where there is car access, then this type of camping allows you to bring a lot of other items.

There are many different types of Coleman camping gear items that will make your next camping trip to the national park an enjoyable one and they are:

How to Choose a Tibetan Bowl

There are two questions my students and clients ask all the time. One is ‘How do I choose a good bowl” and the other is ‘How do I know which bowl is good for a specific chakra’. People also call to obtain a bowl with a certain musical note. My answers to these questions are founded in the Eastern philosophy rather than from a Western mindset, because this better reflects the nature of the instruments as they were intended:

Choosing a good bowl requires a few things.

Quality: Either an eye and an ear for excellent quality- or knowing and trusting someone who knows the bowls and can help you. Many people sell bowls and most are new and machine made to look old. There are several signs of age in a bowl including the shape, the markings inside and out, the thickness of the metal especially on the bottom (thinner is usually older), and the tone. Most lay people will not be able to tell the difference between an antique and a good reproduction. It takes a trained eye. But you should be able to hear the difference. Often even the people who sell bowls in shops and on the internet are not very knowledgeable, so ask a lot of questions. When someone says a bowl is old in this country old can mean 50 years. That is new by my standards. A real old bowl it will be at least 100 years old and more often hundreds of years old. If the vendor doesn’t know the region the bowls were made in, and how the markings help distinguish the type of bowl, they probably don’t know much. Bowls were made in a variety of regions of Tibet and their shape and markings are telling. Some of the bowls have consciousness transformation as their primary intention, and some have physical healing. This can be determined by their shape. There is more about this and photos in my home study course available on my website. A high quality, ancient bowl is an investment. If it is inexpensive and old, then it was probably obtained in a less than honorable manner…A good bowl is an investment and the price can range from $110 for a very small bowl to thousands of $$ for a huge bowl.

On the other side of the coin is the seller who hikes up the price claiming that a bowl is from the 16th or 17th century. If you really want to determine the exact age of a bowl, you have to melt it down- thereby destroying it. Ask where and who authenticates the bowls and how the age was determined and tune into your intuition about the story you are being told.

In Asia, the only way to establish a good and trustworthy relationship with a supplier is over a number of years, and eyeball to eyeball. So the few people who go over themselves to select instruments over a period of years will get the best bowls. If your bowls come from a wholesaler somewhere, w ho purchases large quantities of things you can be sure they are if inferior quality because they probably did not hand select them or have a long term relationship with the supplier. These things DO make a big difference.

Usage: It is useful to determine what you will use the bowl for: ie meditation, grounding, physical healing? Do you want to integrate a few bowls into an existent modality or use them with other instruments in a musical vain, or become a sound healer? The answers to these questions will also impact on your choices.

Tone: The tone of the bowls should linger for quite some time when struck producing several layers of tones you can hear that in turn create overlaying harmonics and overtones. When you sing a bowl (rub the rim with the wooden or leather part of a stick) there are several things to consider. How ‘awake’ the bowl is: some bowls sing right away, and some need to be played for a while and ‘broken back in’. This is neither good or bad but it is important to know so you don’t dismiss a potentially great instrument just because it doesn’t sing right away. When you hear a bowl, listen to your body. If it opens your heart, or moves you deeply in some manner; if you feel tingling all over, or the tone directly impacts one area of your body or grounds you- these are all good signs. Bowls were created for consciousness transformation and healing. You should feel their effect on you.

If you have more than one bowl then it is good to play them together so that their tones harmonize. Eventually, they will anyway, because that is their nature and magic, but why not help the process by starting with bowls that sound great together right away? Note that with several bowls, playing them in one pattern may sound very good and another pattern may not!

Bowls and Chakras. In the west we work with seven chakras. The Tibetans work with five regions of the body. The bowls were created to bring us back to our experience of the interrelationship of all things; thus they are not calibrated to work on any one chakra to the exclusion of the rest. Their very nature encompasses our wholeness. In this way they are very different instruments then the western counterparts- tuning forks and crystal bowls. However there are some bowls whose tone and size lend themselves more naturally to certain areas of the body. Lower tones for grounding, largest bowls by the feet, soothing mid tones around the head, and higher tones over the 3rd eye, etc. There are too many variables to discuss here but you can get some basic principals in my home study course. Having several bowls is wonderful but understand that even if you have only ONE bowl, it will impact all of you, and not just one chakra.

What to Do When You Car Needs a New Engine – Repair it Or Write it Off?

First thing to consider in making this decision is, were you planning on getting another vehicle in the very near future? If the answer to this question is yes then you have to ask the second question. And that is were you going to use the vehicle as a trade in on that next vehicle? If the answer to that is yes then you have to answer a third question. Can you afford to buy another vehicle without using this vehicle as a trade in? If you can answer this question yes, then maybe buying another vehicle is the thing to do.

But in most all cases the answer to one or all of those three questions is great big NO! So let us look at what the reason to ask these questions and what they have to do with the original question. What to do when your vehicle needs a new engine? Repair it or write it off? And the answers to those questions are going to help you determine the best thing to do in your current situation.

The first question is answered no 98% of the time. Why is that? Because most of the time we have no reason to get a new vehicle when the one we are driving is working fine. So if you had no intention of buying another vehicle before your vehicle broke down, why would you suddenly need to buy one now? Unless you are going to go out and buy a brand new vehicle or one that is still under a factory warranty, what are you really doing? You are going to get rid of your vehicle that you know everything about and has been a trusted mode of transportation, for one that you know nothing about and may have more problems than your current vehicle. If it does have more problems that were hidden and it starts breaking down. You will have spent the money you could have fixed that old trust friend with and now have to start putting money into this vehicle. By the time you are done you could be spending much more than if you had just replaced the engine in your original vehicle.

The second question is not answered no to as often as the first question, because if you have answer no to the first question, you had no intention of trading this vehicle in. So if you have answer no because you were not thinking of trading it in, then why would you suddenly want to get another vehicle just because it is expensive to replace the engine. But for all same reasons above the investment in your current vehicle will usually be the a better decision than writing it off.

Finally the answer to the third question is no at least as often as the first question. And if you can afford to buy the vehicle without a trade in and you are planning on buying a brand new vehicle, then writing off this vehicle may be your best decision. But if you are just going to buy some other used vehicle then again we go back to all of the reasons in question one why you should make the investment in your current vehicle. You can not buy any reliable vehicle for less money than it will cost you to put a rebuilt exchange engine into your current vehicle. Sometimes the idea of buying another vehicle is more appealing than fixing the current vehicle. Maybe your eyes are just tired of looking through that same windshield or your seat is tired of sitting in that seat.

Kids and Restaurants: How to Dine Out With a Toddler

One of my favorite times of year is Restaurant Week. If you’re planning to participate or just need some tips about how to dine out with your toddler, here are some tips that should make the process go much smoother. Of course, if you are even reading this, I know that circumstances did not allow you to get a babysitter.

Have a Plan

Going to a nice restaurant (especially for dinner) with a toddler will work best when there is a plan in place. Trust me on this one. My family’s frequent travel lifestyle has allowed us to dine in some of the finest restaurants around the world. But no matter if we are dining in London, Bali, New York or Maui one thing remains the same, we always have a plan in place when dining with the little one. One way to do that is to peruse, the restaurant’s menu online ahead of time so that you can and get an idea of what you would like to order. This will save everyone time and help things run more smoothly.

Dine Early

It is not a good idea to schedule dinner when your little person is sleepy or hungry. Yes you read right. Give your toddler a small snack right before you get to the restaurant so that he or she is not famished waiting for your order to get to the table.

Request a Corner Table

Ask the restaurant host/hostess about getting a seat in a corner or away from most restaurant patrons if possible. Remember, although you were not able to secure a babysitter for your night out, you should try to be courteous to other restaurant patrons that are enjoying their night out. Your time will come.

Set the Rules

Be clear with your toddler about what the rules are early on. Set boundaries and enforce them. For example, keeping your little tike in the booster seat or high chair through-out the course of the dinner is a good rule to enforce. Not only will it keep small fingers out of other people’s food but, it really is not safe for an un-guided toddler to be walking around a restaurant anyway. Candle lit dinners, knives, hot plates and bustling servers can make the restaurant an unsafe place for a toddler to be.

Kids First

Even if the server attempts to take your order first before that of your little one, politely request that you need to order the food of your toddler first. Also indicate that you would like your child’s meal to be served as soon as it is ready. More polished servers will do this automatically; novices however, may need a little reminder. Remember, although many steakhouses and fine dining restaurants may not have kid’s menus, the chef is willing and able to prepare items like chicken fingers, pasta with sauce and other items upon request. You just need to ask.

Come prepared

I always go to even the finest restaurants equipped with three things: a bib, a sippy cup, and a favorite book or quiet toy (such as a plush animal or book). In addition, do not forget to pack essentials like diapers, wipes, pull-ups etc if you don’t want any accidents.

Cut it Short

I hate to say it, but this is probably not the time to order the chef’s “special tasting or prix fixe menu”. A two course maximum is essential unless you are dining at a very kid friendly restaurant or your toddler is more patient than usual.

Be Ready to Call it Quits

At the end of it all, you should be ready to signal for the check and a to-go container at a moment’s notice. As a parent, you will know when your toddler has endured enough of a “good thing”. When that time comes, appreciate it for what it was and head out the door…food in hand of course.

Although I hope that you will not need these tips ( due to having a reliable sitter and back-up sitter on speed dial), following these pointers will make you a pro at dining out with your toddler in no time.

Taking the Plunge – Snow Skiing Tips for the Beginner – Part 2

Now that you’ve taken the big step, booked your ski vacation and reserved your spot in the beginners skiing class (as discussed in Part 1 of the series), its time to take a look what you’ve actually let yourself in for.

So what exactly will your first skiing lesson try to teach you?

Tip # 2 – Learning Control

The most critical skill in learning to snow ski is that of control. Remember, you’re adding a hefty amount of extra length to your feet – being able to control your feet and moving on them will keep your behind dry and your pride intact.

Rather start out with shorter skis than longer ones. Although there are disadvantages such as less glide in using shorter skis, this gives you the opportunity to get the basics right and then upgrade your ski length as your skill improves. Once you have the skis on, use your poles as a balancing device while moving your weight from side to side on your skis. This side to side motion is what will eventually guide you down the mountain slope at a speed that you feel comfortable at. The shortest route to the bottom of the slope is straight down – but this is neither advisable at beginner level, nor necessarily the best way to get to the bottom! So make sure you’re comfortable with shifting your weight from one foot to the other while on the skis.

Probably the most important skill to learn control of is how to STOP without doing yourself some major injury. The easiest and best way to slow your momentum is to point the toes of your skis towards each other and spreading your heels apart – much like a slice of cake. NB – but whatever you do, DON’T let the tips of your skis cross each other – it’s a surefire way of ensuring a wet and uncomfortable tumble.

Another important skill is learning balance. And the best time to practice is while you’re still on flat ground! The trick is to practice balancing on one ski at a time. Lift one ski a couple of centimeters off the snow and shift all your weight to the other leg. Observe how your body adjusts to maintain your balance on just one foot – and remember, your ski-poles are there to help you. Now try skiing on just one leg to get used to being in balance, whether you’re on two feet or only one. You won’t go haring down the mountain on one foot though – this is just an exercise to get you used to your own sense of weight distribution and balance. It is advisable to be comfortable skiing on one leg (practice doing this on both) before you become proficient in skiing on two.

Now that you’ve put on your skies, practiced shifting your weight from side to side, found your balance on your skis and learned how to stop your forward momentum, you’re almost ready to face your first down hill ski. But first, you’ll have to master one more critical skill that everyone needs to learn … how to get up when you fall.

Innumerable sites provide recipes and cooking advice. Countless forums have been set up to aid the novice and give the experts a place to trade tips and recipes. Don’t be afraid to go online and ask questions. Make sure you read what you can before posting questions, but rest assured that there are lots of people out there who are willing to provide advice.

Once you’ve decided to give a gourmet recipe a try, you may find that your local supermarkets don’t have all the ingredients you need. Whenever possible, shop locally but if you can’t find what you need there are a number of online food stores which will ship fresh food to your doorstop at a reasonable rate. Yes, gourmet ingredients tend to cost more but their quality and great taste will make it all worth it when your friends and family are being blown away by your latest excursion into gourmet deliciousness!

When Your Health Insurance Plan Won’t Pay

The very nature of managed care health insurance plans increases the likelihood of a legitimate health insurance claim being denied. Bear in mind that managed care (health maintenance organizations, or HMOs, and preferred provider organziations, or PPOs) exist for the purpose of controlling costs for the health insurance company. Many health care procedures, surgeries, durable medical equipment and drugs, particularly the more expensive ones, require prior authorization from the health insurance plan before the plan will pay. Claims are reviewed to determine “medical necessity” of the claim. Health care services or products deemed “not medically necessary” will almost certainly be denied for payment by the health insurance plan.

Health insurance companies do make mistakes, however, and it’s certainly possible that a covered expense will be denied. What recourse does the health plan member have when one disagrees with the decision of the health plan? Here are some steps to take in dealing with a denial of payment.

1. Review the explanation of benefits (EOB) sent to you from the health insurance company. The EOB should state what services or goods were billed and briefly why benefits were denied.

2. Review your particular health insurance policy. What benefits does the health insurance policy state for the particular service or product? Should the claim be covered according to the policy?

3. Does the health plan have special criteria to be met in order for an particular expense to qualify as “medically necessary” and be considered a covered expense? For example, many managed care plans will cover drugs on their formulary. Other, nonformularly drugs may not be covered at all, or may be covered only if the formulary drugs have been tried and failed. An expensive MRI procedure may only be covered if certain symptoms are present. Check your policy to determine whether the expense qualifies as “medically necessary” by the health insurance company. Your health care provider must submit sufficent documentation to the health insurance plan to justify the need for the expense.

4. Is the health care provider “in-network” (contracted) with your health insurance plan? If not, does your managed care plan cover “out-of-network” (non-contracted) providers? Most HMO plans do not cover “out-of-network” providers; many PPOs will pay for services by “out-of-network” providers, but usually at at lower rate than paid to “in-network” providers.

If, after reviewing the health insurance policy and the EOB, you feel that the claim should have been a covered benefit by the insurance company, you should first request in writing that the insurance company provide you with the information that they used to base their denial of benefits. The health insurance company is required to provide you with this information on request. Review this information carefully. Many times the health insurance company was not provided with appropriate or sufficient documentation from the provider to justify the claim. If this is the case, contact the provider and request that they submit more medical records that support the claim for benefits. It may also be helpful for the provider to write a letter to support the claim in addition to the medical records. Your claim may be resolved in this manner.

All health insurance companies have a process in place by which plan members can appeal the decisions of the health insurance company. If providing further documentation does not resolve the dispute, then an appeal must be filed with the health insurance plan. Your provider may help you with this, and they may not. Read the member handbook and/or policy and follow the procedure for appealing the denial of the claim. Be prepared to submit more documentation to support your appeal. Keeping a record of all interactions with the insurance company is vital. Record all phone conversations and include the name of the person you spoke with, a brief summary of the conversation, and the date and time. File all correspondence sent and received, and have it readily accessible.

Bottom line is that health insurance plans are “for-profit” entities; in business to make money. They look for reasons not to pay. Indeed, their goal is to not pay, increasing their profits and keeping costs down for the members. It’s up to you to ensure that legitimate claims for covered benefits are paid.
